We bill in-network for:

  • All Motor Vehicle / PIP claims in Oregon

  • Blue Cross / Blue Shield / Regence

  • MODA

  • Pacific Source

  • CareOregon (follow up visits require authorization)

  • Kaiser/CHP (for self-referral, exams and adjustments only are covered, call us for more info)

Out of network: we are NOT contracted with these insurances, but depending on your out of network coverage, we may be able to bill for your visits. Be aware that you will need to be aware of your out of network coverage, which sometimes is subject to a deductible.

Insurance Benefit Check Form

Many people have chiropractic coverage under their insurance. Before you come in for your first appointment, as a courtesy to you, we will gladly research your insurance coverage; however, ultimately, you will be responsible directly to The Bodhi Tree Clinic for full payment of your account if your insurance company does not extend coverage for our services. This form will need to be submitted at least 24 hours in advance of your treatment.
